IT-Based Monitoring of Adverse Drug Reactions in Institutional Long-Term Care for People with Dementia

Abstract:
Older care-dependent people living in nursing homes often suffer from a multiplicity of physical and mental health problems. As a result, polypharmacy and resulting adverse drug events (ADE) are common and an important cause of further morbidity (e.g. falls), increased hospital admissions and also mortality. Monitoring the risk of ADE is a challenge in nursing homes worldwide. Computer-assisted monitoring systems for ADE - already common in hospitals to assist physicians - could be a step in the right direction.  In Germany, the situation is additionally complicated because nursing homes typically do not employ a physician, but residents are under the care of their family doctors. The only exception is the “Berlin Model,” whereby in Berlin some nursing homes employ their own physicians. The study aims to evaluate effects of the implementation of a computer-based ADE monitoring system in the nursing homes within the “Berlin Model”.

A longitudinal (2012-2015) cluster-randomised parallel group design will be used to evaluate the effect of implementing a computer-assisted monitoring system for ADE in the nursing homes within the “Berlin Model”. Primary outcomes are quality of life (QUALIDEM), number of falls and ADL (Barthel-Index) of residents.

The study will generate knowledge of how the implementation of a computer-based ADE monitoring system in nursing homes helps to decrease ADE in older people with multimorbidity and simultaneously results in better health.
